April 3-4, 2018: CPCA Introduction to PCMH 2017: Foundational Concepts of the Medical Home
The California Primary Care Association, in partnership with Aliados Health, Arizona Alliance for Community Health Centers, Nevada Primary Care Association, and Community Clinic Association of Los Angeles County, will be hosting trainings on NCQA’s Patient-Centered Medical Home 2017 Standards.
Target Audience: Health Care Professionals or Health Care Decision Makers interested in getting the inside perspective directly from expert faculty from NCQA in evaluating primary care practice and their ability to become Recognized as a Patient-Centered Medical Home by NCQA.
