PHASE Back to Basics

December 7, 2017: PHASE Back to Basics with Dr. Jerry Minkoff

Target Audience: Clinicians and Care Teams

The goal of this session is to strengthen the skills and confidence of registered nurses who are co-managing PHASE patients under standardized procedures. Participants are asked to submit at least one client case in advance. There will be opportunities for Q&A following cases.

December 7 Learning Objectives

  1. Outline a plan to titrate medications for achieving blood pressure goals;
  2. Customize A1c goals for patients using current evidence & clinical considerations;
  3. Select medications & follow recommended timeframes for achieving treatment goals when caring for patients with type 2 diabetes.

Time: 12:00pm-1:30pm
