The goal of this session is to strengthen the skills and confidence of registered nurses who are co-managing PHASE patients under standardized procedures. Participants are asked to submit at least one client case in advance. There will be opportunities for Q&A following cases.

- Outline a plan to titrate medications for achieving blood pressure goals;
- Customize A1c goals for patients using current evidence & clinical considerations;
- Select medications & follow recommended timeframes for achieving treatment goals when caring for patients with type 2 diabetes.
