Sonoma County Health Centers

Sonoma County ranks #7 among 57 counties in California for quality of overall health, demonstrates healthy behaviors including low smoking rates, and one of the lowest percentage of adults reporting fair or poor health.¹

Even with those rankings, the uninsured and underserved populations are in need of access to care. In Sonoma County, Community Health Centers serve 29% of the population² and provide comprehensive healthcare services to the community.

With health centers and clinics in Forestville, Sebastopol, Guerneville, Sonoma, Santa Rosa, Petaluma, Healdsburg, Cloverdale and more, Aliados Health member health centers have 37 sites and 1 wellness education site in Sonoma County. In 2018, these health centers saw over 126,600 patients for over 637,000 encounters. 59% of the patients are on MediCal, 22% of the patients are uninsured and 48% make less than 100% of the federal poverty level.³
