Why the HRSA Health Center Controlled Network (HCCN)?

The Aliados Health Health Center Controlled Network (HCCN) operates as a cooperative agreement with the U.S. Health Resources and Services Administration (HRSA) “to support health centers in leveraging Health Information Technology (HIT) and data analytics to enhance delivery of affordable, accessible, and high-quality primary health care, with a specific focus on”*:

  • Data management and analytics
  • Interoperability of systems and digital health tools
  • Data Modernization
  • Cybersecurity
  • Artificial Intelligence

PROGRAM

What We Do

Aliados Health HCCN convenes peer groups for Electronic Health Records users, Data Leads, and Cybersecurity/IT peers to support the needs of our participating health centers and to offer a platform to share promising practices and work on resolutions to issues and to provide aggregated data dashboards for the health centers.

Aliados Health HCCN works with the health centers to collaborate and help facilitate new services and contracts for Health Information Technology, Analytics Platforms, Cybersecurity and Health Information Exchange.

Aliados Health HCCN provides technical assistance, trainings and toolkits on current initiatives, i.e. use of analytics reports, roll out of Evidence Based Care tools (guidelines, protocols and clinical decision support tools), and topic specific sessions including areas within SQL Coding, Health Information Technology and Health Coaching.

Who We Serve

We serve twenty-one participating health centers and nearly 446,000 health center patients from seventeen Northern California counties:

Marin and San Francisco Counties: Marin Community Clinics, Marin City Health and Wellness, and Ritter Center

Sonoma County: Alexander Valley Healthcare, Alliance Medical Center, Petaluma Health Center, Santa Rosa Community Health, Sonoma Valley Community Health Center and West County Health Centers

Yolo, Napa and Solano Counties: CommuniCare + OLE Health Centers and Winters Healthcare Foundation

Mendocino County: Anderson Valley Health Center and Long Valley Health Center

Humboldt County: Redwoods Rural Health Center

Placer, Sierra, Nevada and Yuba Counties:  El Dorado Community Health Ceneter and Western Sierra Medical Clinic

Contra Costa County: Brighter Beginnings Family Health Center

Santa Clara County: Indian Health Center of Santa Clara Valley

San Joaquin County and Solano County: Community Medical Center

Shasta County: Shasta Community Health Center

Siskiyou County: Shasta Cascade Health Center

Objective 1: Data Management and Analytics

  • Support growth and engagement in Community Information Exchange
  • Templates, policies, procedures and workflows for information sharing that adheres to Federal and California data sharing and privacy regulations
  • Integration of Health Information Exchange data sources into Population Health Management tools
  • Support of bidirectional exchange and integrated referral and consultation information

  • Update cybersecurity policy and procedures
  • Improve ability to vet health IT vendors and partners to assess their cybersecurity practices and ensure they meet security standards
  • Cybersecurity workgroups and training to recognize and prevent social engineering attacks
  • Expand implementation of Multifactor Authentication

Objective 5: Artificial Intelligence

  • Advance implementation of ambient listening at PHCs
  • Support of vetting Artificial Intelligence tools for health centers
  • Training on fair, appropriate, valid, effective and safe use (FAVES) of Artificial Intelligence
