What We Do
Aliados Health provides technical assistance and infrastructure for health centers to participate in health information exchange (HIE) to facilitate care coordination, specialty referrals, and transitions of care.
Direct and Query-Based HIE:
- Query-based HIE allows care team members to query the exchange for patient records to facilitate treatment among providers utilizing disparate health information systems, including when patients use an outside system such as an emergency room or hospital. Query exchange allows for admit / discharge / transfer (ADT) feed alerts within electronic record systems and exchange of continuity of care documentation.
- Aliados Health has formed Redwood Community Health Information Exchange (RCHIE), a private query-based HIE hub, to facilitate health center access to timely data sharing, meeting interoperability goals, and achieving economies of scale to connect with hospitals, regional health information exchanges and priority partners. Eight Aliados Health health centers populate records into RCHIE which contains close to 250,000 continuity of care records and has exchanged more than 5 million coordination of care documents.
- Community providers serving shared patients may access RCHIE records for continuity of care through a web portal. Complete the RCHIE Access Request Form to request access.
- Aliados Health, under the CalHOP program, has finalized connection to SacValley MedShare in Spring 2021, this expands the patient query reach to an additional 18 counties in Northern California. Please visit the SVMS website for additional information.
- Aliados Health supports health centers to engage in direct HIE through Aliados Health’s provider to provider community directory. Direct HIE allows for secure provider to provider communication, referral requests between primary care and specialists for services, and integration and coordination between primary care and with county behavioral health departments for treatment of shared behavioral health patients.
HIE Steering Committee:
Aliados Health’s HIE Steering Committee meets bi-monthly with the objectives of making strategic decisions to ensure all organizations have continuous access to information that is supportive of the common goal of providing the most complete care possible. This includes discussion of running issues and successes related to HIE and sharing of best practices. The committee is made up of a representative from each health center who has the authority to make decisions related to health center interoperability connections .

California Data Exchange Framework (DxF)
As the COVID-19 pandemic pushed virtual care to the front of primary care, it highlighted not only the digital divide safety net patients face but also the long-held struggle of sharing patient information across entities in a technologically efficient manner. Remember trying to figure out COVID testing and result reporting?
In 2021, recognizing the landscape and needs, the California legislature passed AB133 which requires all health and human services organizations, including “physician organizations” to: “exchange health information or provide access to health information to and from every other entity…in real time as specified by the California Health & Human Services Agency pursuant to the California Health and Human Services Data Exchange Framework (DxF) data sharing agreement for treatment, payment, or health care operations.”
As Health Centers continue to move into whole person & expanded care models supporting addressing Social Drivers of Health, the DxF plays an important role in accelerating and expanding the exchange of health information among healthcare entities, government agencies and social service programs beginning in 2024.

HIE Trainings
Providing access to a Health Information Exchange is a single piece of the interoperability puzzle. Just having access to information is not enough, Aliados Health has created end-user training videos to support Health Centers in optimizing shared information.

Information Blocking Resources
The 21st Century Cures Act, signed December 13, 2016, by President Obama, promotes and funds among several other things a number of provisions to improve healthcare IT—most notably, in relation to nationwide interoperability and information blocking. It places strong emphasis on providing patients access to their electronic health information that is “easy to understand, secure and updated automatically.”

How an HIE helps you.
Most healthcare organizations are closed evenings, weekends, and holidays, an HIE allows providers to access information 24 hours a day. Many northern Californians travel for personal and professional reasons. Emergencies can happen far away from your primary care and medical records. Sharing test results, coordinating care, and providing timely feedback improves the quality of care.

RCHIE is also connected to SacValley MedShare regional HIE, you can obtain a current list of SVMS participants at http://sacvalleyms.org. Participating providers (such as doctors, hospitals and pharmacies) and health insurers (for their own insureds or for insureds in group health plans they administer), public health agencies, and their staff and contractors will have access for treatment, payment, and public health purposes.
How does sharing health information improve my care?
Health care providers need your complete health information to accurately diagnose and treat you. Each of your providers may have different portions of your medical record. If they can access each other’s records and see more complete health information, they can provide you with better care. Sharing your health information can help reduce medical care costs by eliminating unnecessary duplication of tests and procedures.

PRIVACY AND SECURITY
Aliados Health follows all the same state and federal HIPPA privacy regulations as your provider and hospitals. Your providers already share patient information necessary to provide safe and timely care. Phone calls, faxes, and postal mail are not the fastest or safest methods for communicating.

The Aliados Health health information exchange was created to allow your providers of care to share pertinent medical information about you which is all secured and protected in accordance with the Health Insurance Portability and Accountability Act.

OPTING OUT RISK EXPLANATION
If you do not want anyone – including your healthcare providers or any emergency department – to have access to your health information in Aliados Health HIE, please complete the opt-out form below, place it in a secure envelope, and return it to Aliados Health at the address provided. By mailing the completed opt-out form, you are requesting that Aliados Health no longer permit your health care providers – including emergency care providers – to have any access to your health information contained in the HIE. We offer this option as it is desired by some patients. We do not recommend opting-out. It is dangerous for physicians to treat you without knowing your health problems, medications and allergies. If you elect to opt-out, your request will be acted upon within approximately five (5) business days after receipt by Aliados Health, and will only apply to access after such date. You are welcome to opt back in at any time by completing an Opt-Out Revocation Form that can be downloaded from http://rchc.net. However, any opt-back-in request from you may take up to five (5) business days after receipt by Aliados Health to be in effect. Also, Aliados Health may not have current health information to make available to your health care providers for some time after the effective date.
