Quality Improvement (QI) Aliados Health is designed and organized to support its mission, values, and goals. The QI Plan reflects a commitment to support the design, implementation, and evaluation of systematic and continuous actions that optimize the delivery of Triple Aim. At the core of our QI work is our Population Health Improvement Framework. The framework promotes health equity, and workforce satisfaction and aims to result in measurable improvement in health care services and population/health outcomes across Aliados Health member community health centers.
The QI team supports community health centers with equity-focused continuous QI efforts by sharing best practices and evidence-based improvement strategies and providing training and technical assistance opportunities. The team also supports health centers with grant-funded projects to advance health initiatives.

Aliados Health’s approach to clinical quality improvement:
Aliados Health utilizes the equity-centered Population Health Improvement Framework for clinical quality improvement which seeks to improve care and outcomes through:
Shared Clinical Guidelines – Health center medical leaders agree upon clinical guidelines based on evidence-based care and health equity. Guidelines align with health center and Aliados Health’s goals and initiatives.
Supportive Tools – Aliados Health and Health Centers develop and share policies, procedures, protocols, and health-IT-based tools that support health center implementation of the clinical guidelines. This also includes clinical decision support and analytics such as recalls, alerts, order sets, templates, and standard reports.
Peer Learning Communities – Aliados Health advances quality and equity through peer learning opportunities including workgroups, podcasts, sharing promising practices, meetings, conferences, and training. Aliados Health’s QI-led functional peer network meetings: Medical Directors, Dental Directors, QI Leads, HELC, SDOH Workgroup.
